A great original Treasury Department issued order form for Opium or Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations!  This 1923 issue (issued in 1930 to Ellis) was recently shown on the History Channel during a program on illegal drugs. Please see photos!
This form is ornate with a beautiful poppy graphic on the top corner. It was issued to L. (Luther) C. Ellis Drug Co., of Glasgow, KY. An order for Pantopon was placed on February 10, 1932.  This form was kept by the doctor/pharmacist for his/her records.
The top original was sent to Geo. J. Ellis, also in Glasgow, KY, to be filled.
The form is large, 10 3/4" by 8 1/2", is watermarked, "USIR", has a corner bend, a couple other bends and a few carbon rubs, otherwise, condition is excellent...a beautiful document!
